Medication errors are a leading cause of preventable hospitalizations. Our registered nurses perform comprehensive medication reconciliation — reviewing every prescription, over-the-counter medication, and supplement. We educate patients and families on each medication's purpose, dosage, timing, and potential side effects, then monitor adherence and response over time.

Medication management is a covered skilled nursing service under Medicare Part A when medically necessary and ordered by a physician.
